With the recent development of competency frameworks and clear career tracks for pharmacists being pioneered at Singapore General Hospital, along with the imminent introduction of a specialist register, this is a very exciting time to be a clinical pharmacist at SGH!

The Neurology Pharmacists provide pharmaceutical care as a crucial member of a multidisciplinary health care team that includes neurologists, rehabilitation medicine physicians, nurses and other allied health professionals. Key aims of our Neurology pharmacists are to maximise the cost-effectiveness of pharmacotherapy in patients with cerebrovascular disorders (e.g. stroke), neurodegenerative disorders (e.g. Alzheimer’s and Parkinson’s diseases) and epilepsy. Neurology Pharmacists will also minimise drug-related complications and improve their patients’ long term outcomes by optimising their drug therapy.

Our Neurology Pharmacists’ guide the use of antiepileptic drugd (AED) at SGH. Our Neurology Pharmacists are acknowledged as drug experts, and we work closely with the medical team ensure appropriate use of AEDs in our patients. Our involvement includes, among others, the monitoring of AEDs with a narrow therapeutic index e.g. Phenytoin, Valproic Acid, Carbamazepine and Phenobarbitone, in order to reduce seizure frequency and severity, as well as to avoid adverse drug events. We regularly conduct physician education with regard to dosing and therapeutic drug monitoring of AEDs, We also collaborate on research e.g. study on prophylactic antiepileptic drug dosing to reduce post-operative seizure incidence in neurosurgery patients.

Neurology pharmacists also play an important role in assessing outcomes and recommending when the use of 2nd generation AEDs. Review of our patient’s clinical response in relation to drug levels, and monitoring of pertinent laboratory data e.g. hepatic enzymes ensures safe and efficacious use of these drugs. In addition, appropriate patient counseling on the uses and effects of these drugs helps to increase adherence and persistence with AED therapy, and timely identification of drug-related problems such as drug interactions and adverse drug effects.

Role

Our Neurology pharmacists will identify, resolve, and prevent drug-related problems, recommend and monitor patient drug therapy. They conduct numerous pharmacy interventions as Neurology and Neurosurgery account for 13% of all pharmacist interventions in SGH, a number of which resulted in avoidance of a serious adverse drug reaction, drug-drug interaction, or major contraindication.

Active participation in regular Neurology clinical rounds and provision of accurate information and evidence-based recommendation, in a timely manner is required too. Similarly, a Neurology pharmacist must be able to educate the general public, patients, students and fellow healthcare workers on safe and effective medication use via various communication channels. Documenting the outcomes of patient care, and overseeing drug distribution activities are also part of the job. In addition, the Neurology Pharmacist advances the knowledge-base in Neurology by performing relevant clinical and/or translational research. The Neurology Pharmacist is expected to support and participate in the SGH mission of providing quality patient care, education, and research.

As we expand the current scope of clinical services we provide, we are looking for motivated and dynamic Pharmacists with a strong interest in the area of Neurology to join our team of healthcare professionals.

Requirements

· Basic degree in Pharmacy

· Registerable with the Singapore Pharmacy Council

· Minimum 3 years of clinical working experience in the management of neurologic disorders with demonstrated competency in this area of practice

· Relevant post graduate degree and/or clinical certification/competency is preferred
