With the recent development of competency frameworks and clear career tracks for pharmacists being pioneered at Singapore General Hospital, along with the imminent introduction of a specialist register, this is a very exciting time to be a clinical pharmacist at SGH!

The Respiratory Pharmacists provide pharmaceutical care as a crucial member of a multidisciplinary health care team that includes respiratory physicians, therapists, psychologists and nurses. The key aims of a Respiratory pharmacist are to maximise the cost-effectiveness of medication use in patients with acute or chronic respiratory diseases such as asthma, COPD and respiratory tract infections, to actively promote smoking cessation, as well as to minimise complications of drug therapy and to optimise patients’ long-term outcomes.

One of the core activities our Respiratory Pharmacists are involved in is the provision of care to asthma sufferers. Our knowledge of the pathophysiology, trigger factors and clinical manifestations of asthma, plus our expertise in pharmacotherapy, places us in an excellent position to assist the medical team manage and monitor patients receiving asthma treatment. In particular, our Respiratory Pharmacists ensure the appropriate use of bronchodilators and corticosteroid inhalers in both inpatient and outpatient settings.

At the outpatient Asthma Clinic, pharmacists and respiratory physicians provide detailed patient education as part of a coordinated care programme. This programme includes individualised patient assessments, and a patient-specific educational and asthma action plan which also includes diaries and checklists when appropriate. Asthma Action Plans can be symptom-based or peakflow-based, and include the prescription of a standby course of oral prednisolone for severe exacerbations.

In the inpatient setting, asthma education also takes place alongside root cause analyses on the cause of the exacerbation and hospital admission. We also teach inhaler technique in patients with COPD, and can provide smoking cessation advice. Our Respiratory Pharmacists may also be trained to cover the inpatient Medical Intensive Care Unit.

Role

The Respiratory pharmacist will identify, resolve, and prevent drug-related problems, and recommend and monitor patient drug therapy. Active participation in regular Respiratory clinical rounds and provision of timely and accurate drug information is required too. Similarly, a Respiratory pharmacist must be able to educate the general public, patients, students and fellow healthcare workers on safe and effective medication use via various communication channels. Documenting the outcomes of patient care, and overseeing drug distribution activities are also part of the job. In addition, he/she is to advance the knowledge-base in Respiratory Medicine by performing relevant clinical and/or translational research. Our Respiratory Pharmacists support and participate in the SGH mission of providing quality patient care, education, and research.

As we expand the current scope of clinical services provided, we are looking for motivated and dynamic Pharmacists with a strong interest in the area of Respiratory Medicine to join our team of healthcare professionals.

Requirements

§ Basic degree in Pharmacy

§ Registerable with the Singapore Pharmacy Council

§ Minimum 3 years of clinical working experience in the management of Respiratory diseases with demonstrated competency in this area of practice

§ Relevant post graduate degree and/or clinical certification/competency is preferred
