With the recent development of competency frameworks and clear career tracks for pharmacists being pioneered at Singapore General Hospital, along with the imminent introduction of a specialist register, this is a very exciting time to be a clinical pharmacist at SGH!
The Transplant Pharmacists provide pharmaceutical care as a crucial member of a multidisciplinary health care team that includes physicians, surgeons, transplant coordinators, nurses and social workers. The key aims of a Transplant Pharmacist are to maximise the cost-effectiveness of medication use in renal, liver, heart and lung transplant patients in both pre and post-transplant setting, to improve graft success and healthy productive patients. Transplant Pharmacists also are involved in minimising the complications of various medications used in the transplant setting, and optimizing the recipients’ long term outcomes. While our Renal Transplant pharmacists have an established role in SGH, our Liver Transplant Pharmacist played a key role in the recent dual heart & liver transplant performed at SGH, which was a world first.
The following case demonstrates our Transplant Pharmacist’s role in guiding the use of immunosuppressant agents and prophylactic antiviral drugs :
A transplant recipient who was CMV Ig positive developed CMV antigenemia post-transplant. The immunosuppressant drug regimen was adjusted. Mycophenolate was discontinued while maintaining prednisolone, with tacrolimus titrated to a therapeutic level ~10?g/L. Valganciclovir was given per protocol, however just before completion of the 2-week course, the recipient developed neutropenia with ANC <500. Valganciclovir was determined to be the root cause and it was immediately discontinued. The recipient went on to develop has severe progressive leucopenia and thrombocytopenia. While awaiting investigation result of CMV PCR and the potential risk of opportunistic infection, our Transplant Pharmacist, who had been reviewing the pharmacotherapy closely, recommended that foscarnet be used due to its lower myelosuppressive effects. She also advised that prednisolone dose could be tapered down. Renal function and tacrolimus trough level were very closely monitored. After 14days, the patient’s WBC and platelet count recovered to normal range. He is now clinically well and discharged with Valganciclovir. Transplant Pharmacists were able, in this instance, to closely monitor a difficult post-transplant situation over the whole course of treatment together with the medical team, to keep the balance between the benefits of anti-rejection and risk of opportunistic infection.
Role
Our Transplant pharmacists are expected to identify, resolve, and prevent drug-related problems, and recommend and monitor patient drug therapy. Active participation in regular transplant clinical rounds and provision of timely and accurate drug information is required too. Similarly, a Transplant pharmacist must be able to educate the general public, patients, students and fellow healthcare workers on safe and effective medication use via various communication channels. Documenting the outcomes of patient care, and overseeing drug distribution activities are also part of the job. In addition, he/she is to advance the knowledge-base in Solid Organ Transplantation by performing relevant clinical and/or translational research. The Transplant Pharmacist is expected to support and participate in the SGH mission of providing quality patient care, education, and research.

As we expand the current scope of clinical services we provide, we are looking for motivated and dynamic Transplant Pharmacists to join our dedicated team of healthcare professionals.
Requirements
§ Basic degree in Pharmacy
§ Registerable with the Singapore Pharmacy Council
§ Minimum 3 years of clinical working experience in Solid Organ Transplantation with demonstrated competency in this area of practice
§ Relevant post graduate degree and/or clinical certification/competency is preferred
